Real Ale has a final fermenting stage in the cask, Ungespundetes has finished its fermenting process and after that its lagering. Fermentation is typically finished before the lagering, leaving no fermentables in the beer, the beer is just unfiltered after the lagering. Lagering is necessary to get rid of the diacetyl, a by-product of the (bottom)fermentation. The yeast strains used for top fermented beers (ales) produce typically far less diacetyl, although some ale yeast strains also produce diacetyl when they go into an aerobic fermentation stage (i.e. with oxygen). If there are still fermentables in a Kellerbier/ungespundetes, and the yeast in the beer is exposed to air (oxygen), it is likely that diacetyl will very quickly develop. "Auf dem Keller" or in the Gaststätte with its typically high turnover rate it (and the right selection of size of the fass, usually emptied in a couple of hours, and with no leftovers at the end of the day) is not really a problem, on CAMRA festivals (where a fass can last more than one day, or is tapped halfway a session and will last until the next day) it *is* a problem.
