Cask Ale in US

I don't know about the Pacific Northwest or New England, but the pubs in Southern California who do regularly have cask ale all use cask breathers -- the beer doesn't move fast enough for it to make sense otherwise. Part of that is un-familiarity but also is partly because pubs that are likely to have a couple of handpumps also have a large number of (keg) taps.
